This portal is for Rhode Island EOHHS providers to order, replace or return FVV devices used for Electronic Visit Verification (EVV) services.

Cost: Devices are provided at no cost to providers

Shipping: Shipping is free of charge on all device orders

Replacement Devices: Replacements will be shipped once the damaged device has been received.

Orders: Orders are linked to the email address associated with your Shopify login. Please make sure you're signed in with the correct email before placing an order.

Device Return Policy

When the device is no longer needed, it is the provider’s responsibility to return it. If you need assistance with a return, please visit the Returns and Replacements Page above and follow the provided instructions.
